Healthlincs partners with local & national agencies to provide care coordination services for patients and program enrollees. Our care coordination support includes linking individuals to healthcare resources, services, processes, and systems to improve healthcare outcomes and meaningful living. Our experienced healthcare professionals put patients' & care team needs first. We are proud to provide a high quality level of clinical & community care support, with a strong commitment to health & wellness. Our goal is to successfully assist with coordinating care needs.

About the Care Resource Fund

Did you Know? ...  

About 66% of Americans report they are very, somewhat or a little concerned they won’t be able to afford medical care; and this has worsened with the pandemic? 

Healthlincs and its partners provide care management for special needs children and adults with chronic illnesses, and sometimes their insurance does not cover their healthcare needs - and certainly no support to address social determinants of health.

The Healthlincs Care Resources Fund was started in 2020 to help individuals we serve with financial assistance to purchase services or products that are not covered by their insurance. Just promoting healthy choices won't eliminate these and other health disparities; we also need to address social determinants of health. 

"Social determinants of health (SDOH) have a major impact on people’s health, well-being, and quality of life. Examples of SDOH include: Safe housing, transportation, and neighborhoods; Racism, discrimination, and violence; Education, job opportunities, and income; Access to nutritious foods and physical activity opportunities; Polluted air and water; & Language and literacy skills. SDOH also contribute to wide health disparities and inequities. For example, people who don't have access to grocery stores with healthy foods are less likely to have good nutrition. That raises their risk of health conditions like heart disease, diabetes, and obesity — and even lowers life expectancy relative to people who do have access to healthy foods.

The Team


Patients & Caregivers

At Healthlincs we believe that patients and their families are at the center of the care team services we provide. Our coordinators support the development and accomplishment of the goals jointly decided by the patients (their caregivers) and the care team.

Healthlincs Care Coordinators 

Our care coordinators are experienced nurses and licensed certified and community health workers. Our care coordinators are passionate about the helping patients navigate the healthcare system together and working hand-in-hand with the primary care providers. They provide tailored care services for patients within and as they transition across the care continuum. 

Administration & Management Staff

Our administrative/management staff utilize a holistic approach that includes a thorough knowledge of industry standards, compliance requirements, emerging technologies, model analysis, and care professional orientation. Our experts include: project mangers, quality improvement specialists, communication specialists, and analysts.
