Healthlincs care delivery transformation model is an advanced primary care model designed to support practices with care transformation implementation. We apply tailored, cost-effective methodologies to ensure a successful care transformation.
As all practices have unique capabilities & opportunities for improvement, we meet partner practices where they are & work closely with them on their terms to facilitate innovative approaches that result in value-based care delivery.
Healthlincs (in collaboration with the partner practice) conducts a baseline practice assessment using our customized practice assessment (PA) tool to assess the practice’s level of transformation across the key primary care functions and drivers. As assessment of the care management processes also identifies high performing care processes and areas of improvement.
Following the assessment, Healthlincs develops a recommended practice-specific approach &roadmap for care transformation outlining People, Processes, and Technology – and integration/handoffs between the partner practice staff and our healthlincs staff
Healthlincs provides care management resources to support partner practices. Other services such as change management & process improvement, are offered to help implement tests of change using PSDA methodology. Our goal is to keep things simple & uncomplicated. Healthlincs works closely with our partner-practices to help maintain a culture of improvement to sustain successful change. We participate in practice team huddles and review sessions. We also use actionable data, to review care delivery standards. We help practices establish a standard operating procedure (SOP) manual to help sustain efficient care delivery processes.
