The Healthlincs Care Resource Fund, established in 2020, provides financial assistance to help individuals access essential services or products not covered by insurance. The Challenge

Millions of Americans struggle to afford healthcare because insurance doesn’t cover everything? Rising costs have left nearly 29 million people “Cost Desperate,” unable to access quality care. Gaps in coverage and social factors like income, housing, food access, and transportation make it even harder. Minority and low-income households are hit hardest—25% of those earning under $24,000 can’t afford care. These challenges deeply impact overall well-being.

Our Approach

At Healthlincs , we combine care coordination, community partnerships, technology, and culturally responsive support to eliminate barriers and improve access to essential health services. Our person‑centered model focuses on early intervention, continuous support, and empowering individuals to take control of their health and overall well‑being.

Children & Youth Mental Wellness K-12

Healthlincs is leading statewide efforts to respond to the growing youth mental health crisis through its Mental Health Wellness Continuum (MWC) Program, funded through a multi‑million‑dollar grant from the Maryland Community Health Resource Commission.

The program delivers trauma‑informed care, school‑based therapy, crisis response, and early‑intervention support designed to meet students where they are. Through close collaboration with schools, families, and behavioral health providers, Healthlincs ensures that children and youth receive timely assessments, consistent follow‑up, and the ongoing emotional wellness care they need. This comprehensive approach strengthens resilience, builds coping skills, and helps students thrive both academically and personally.

Women’s Health & Chronic Disease Management

Healthlincs addresses chronic diseases with tailored support to diabetes, hypertension, and heart disease, through a coordinated care model that blends health education, behavioral health support, and personalized care planning. The program helps women understand their risks, build sustainable lifestyle habits, and access culturally appropriate medical and social support systems.

Through comprehensive care coordination, preventive education,
and ongoing wellness coaching, women receive the tools and guidance needed to manage long‑term health conditions effectively. This approach strengthens health outcomes, reduces complications, and empowers women to take charge of their well‑being at every stage of life.

Individuals with Disabilities

Healthlincs expands access to behavioral health services for individuals with disabilities through inclusive case management models built on accessibility, dignity, and personalized support. The program focuses on understanding each person’s unique needs and removing barriers that often make care difficult to access. Coordinated service navigation, provider collaboration, and tailored behavioral health support ensure that individuals receive compassionate, consistent, and equitable care. Integrating accessibility practices at every stage of service empowers individuals with disabilities to maintain stability, strengthen independence, and experience meaningful improvements in mental wellness and overall quality of life.

Our work has led to improved health outcomes, expanded access to mental health services, and significant reductions in care gaps, ensuring that community members receive timely, person‑centered support when they need it most.  

Across our programs including behavioral health, care management, wellness support, and targeted initiatives for children, youth, women, and individuals with disabilities. We have consistently increased system navigation success, enhanced continuity of care, and supported long‑term well‑being for those facing complex social and medical challenges. Our coordinated approach has helped beneficiaries overcome barriers such as food insecurity, transportation limitations, mental health access issues, and the fragmentation often experienced within the healthcare system. 

These impacts are reinforced by strong program data demonstrating improved well‑being, reduced emergency room utilization, and expanded support to thousands of residents annually reflecting the meaningful difference Healthlincs makes in communities with the greatest need.
